Agreement Form for Completion of Requirements

For “special circumstances” such as: application for reinstatement, withdrawal from Honors College, waiver of the honors activity requirement for a quarter, or adjustment of the required number of honors units from 12 to 9. Students must complete the form with a statement of why they submitted the petition. The completed form must be submitted to hcollege@uic.edu for review. Each petition submitted will be reviewed by Associate Dean Stacie McCloud and students will be notified of the decision within one week of submitting their petition. If further discussion is required, students will be invited to meet with Dean McCloud to make a final decision. The Honors College Capstone Completion Form is required for students enrolled in HON 322 who have completed their capstone project at the end of the semester. The capstone filling form is the fourth (#4) and last form in the standard series of 4 forms related to synthetic stone. The form must be approved by the project supervisor and the faculty fellow. The form completed with supervisor and colleague approvals is available for Honors College until the last day of class.

In addition, students must submit a copy of their completed synthesis project, more information can be found on the HON 322 Blackboard website. To access a Capstone Completion Form, the approved Capstone Proposal Update Form must be received by Honors College. Use the Honors activity reporting system to access the Capstone fill-in form. The honors activity completion form is required for any student enrolled in HON 222. To receive a diploma form, your agreement form must have been received by Honors College. After initiation, the completed form is forwarded to the activity manager for approval. The approved form must be received by Honors College by the last day of the last week. Log in to the Honors activity reporting system to access the completed form. All Honors College students are awarded a Faculty Fellow at the beginning of their second year.

Students who wish to apply for a specific Fellow or request a change to their Fellow assignment must complete this form. Applications will be reviewed between the last week and the end of week 1 of the following semester (p.B. an application submitted in week 2 of the spring semester 2019 will be processed for the fall of 2019). The Honors College Capstone agreement form is required for any student enrolled in HON 322. It is the first (#1) of a series of 4 forms related to synthetic stone. This is in place of the Specialization Activity Agreement form (above) and must be completed at the beginning of the semester where a student starts working on the synthesis project. Once the student has submitted the summary agreement form to HARS, the project supervisor will be notified for electronic approval within one business day. Only after the project manager has approved the project will the fellow be notified for electronic approval. The form must be created in HARS before the end of the third week of the semester. Submit this form 45 days before graduation (usually at the end of March). It does not replace the requirements of your university college`s diploma statement. The Honors College Capstone Progress Report is required for students enrolled in HON 322 who have NOT completed their capstone project by the end of the semester (Note: Please remember that capstones typically consist of 2 semesters). Therefore, this form is usually the second (#2) in a series of 4 forms related to synthetic stone. Note that the form only needs to be approved by the project manager. The approved Capstone Progress Form must be obtained from Honors College until the last day of class of the first semester of the synthesis thesis. To receive a capstone progress report form, your approved capstone agreement form must have been obtained from Honors College. Use the Honors activity reporting system to access the Capstone progress report form. Honors College`s De capstone proposal update form is required for students enrolled at HON 322 who have NOT completed their synthesis project in the previous semester. The form must be approved by the project supervisor and the faculty fellow.

The form must be created in HARS no later than the end of the third week of the semester. To receive a Capstone Proposal Update Form, Honors College must have received the signed Capstone Agreement Form AND the Progress Report Form. Use the Honors activity reporting system to create the Capstone proposal update form. The honoris activity agreement form is required for each student enrolled in HON 222. Find the activity you want to perform and fill out the appropriate agreement form. Once the student has submitted the activity to HARS, the activity supervisor will be notified for electronic approval within one business day. Only after the activity supervisor has approved your activity will the fellow be notified for electronic approval. The agreement form must be drawn up at HARS no later than the end of the third week of the semester.
